HIPAA Notice of Privacy Practices

This Notice of Privacy Practices (“Notice”) describes how medical information about you may be used and disclosed and how you can get access to this information. Please review it carefully.
1) Our Commitment to Your Privacy
AWS Services & Nursing Inc. (“AWS,” “we,” “us,” or “our”) is committed to protecting the privacy of your health information. We are required by law to:
- Maintain the privacy and security of your protected health information (“PHI”);
- Provide you with this Notice of our legal duties and privacy practices;
- Follow the terms of the Notice currently in effect; and
- Notify you if a breach occurs that may compromise the privacy or security of your PHI.
PHI includes information that identifies you and relates to your health condition, healthcare services, or payment for healthcare.
2) How We May Use and Disclose Your PHI
The following are common ways we may use and disclose your PHI. Not every use or disclosure is listed.
A) Treatment
We may use and share your PHI to provide, coordinate, or manage your care (including telehealth services), such as communicating with you and, when appropriate, coordinating with other healthcare providers.
B) Payment
We may use and share your PHI to bill for services and receive payment, when applicable (for example, verifying information or responding to payment-related inquiries, consistent with law).
C) Healthcare Operations
We may use and disclose your PHI to support business operations, such as quality assessment, staff training, licensing, auditing, compliance, and improving services.
D) Appointment Reminders and Communications
We may contact you to remind you of appointments, provide instructions, or discuss services. We may contact you by phone, email, or text depending on the contact information you provide and your preferences.
E) Business Associates
We may share PHI with third-party vendors (“Business Associates”) that perform services on our behalf (e.g., secure form platforms, IT services, or telehealth technology). Business Associates are required to protect PHI and use it only as permitted by law and their agreements with us.
F) As Required by Law
We may disclose PHI when required by federal, state, or local law (e.g., to the U.S. Department of Health and Human Services for compliance investigations).
G) Public Health and Safety
We may disclose PHI for public health activities, reporting, preventing disease, or to reduce a serious threat to health or safety, as permitted by law.
H) Health Oversight Activities
We may disclose PHI to oversight agencies for authorized activities such as audits, inspections, investigations, and licensure actions.
I) Legal Proceedings
We may disclose PHI in response to a court order, subpoena, or other lawful process, as allowed by law.
J) Workers’ Compensation
We may disclose PHI for workers’ compensation claims as permitted by law.
3) Uses and Disclosures That Require Your Written Authorization
We will obtain your written authorization for uses and disclosures not described in this Notice, unless otherwise permitted by law.
In general, we will not use or disclose your PHI for:
- Marketing purposes (where required by law),
- Sale of PHI,
without your written authorization.
You may revoke your authorization at any time in writing, except to the extent we have already acted on it.
4) Your Rights Regarding Your PHI
You have the following rights under HIPAA (some rights may be subject to limited exceptions):
A) Right to Get a Copy of Your Health Records
You may request access to or a copy of your medical records and other PHI we maintain about you. We may charge a reasonable cost-based fee as permitted by law.
B) Right to Request Corrections
You may ask us to correct PHI about you that you believe is incorrect or incomplete. We may deny your request in certain circumstances, but we will explain the reason in writing.
C) Right to Request Confidential Communications
You may request that we contact you in a specific way (e.g., only by phone, or at a different number/address).
D) Right to Request Restrictions
You may request restrictions on how we use or disclose your PHI for treatment, payment, or healthcare operations. We are not required to agree to all restriction requests, but we will comply if required by law.
E) Right to Get a List of Disclosures
You may request an accounting (list) of certain disclosures of your PHI made in the last six years, excluding disclosures for treatment, payment, healthcare operations, and certain other exclusions.
F) Right to Get a Paper Copy of This Notice
You may request a paper copy of this Notice at any time, even if you agreed to receive it electronically.
G) Right to Choose Someone to Act for You
If you have a legal guardian, healthcare power of attorney, or another authorized representative, that person may exercise your rights after providing appropriate documentation.
5) Our Duties
We are required to:
- Protect the privacy and security of your PHI;
- Provide this Notice explaining our practices and your rights;
- Follow this Notice as currently in effect; and
- Notify you following a breach of unsecured PHI as required by law.
6) Complaints
If you believe your privacy rights have been violated, you may file a complaint with us and/or with the U.S. Department of Health and Human Services (HHS). You will not be penalized or retaliated against for filing a complaint.

7) Changes to This Notice
We may change the terms of this Notice at any time. Changes will apply to all PHI we maintain. The updated Notice will be available on our website and upon request.
8) Website and Third‑Party Tools (Important)
Our website may link to or embed secure third-party tools for scheduling, forms, record submission, or online services. Those tools may have their own privacy practices. When those tools handle PHI on our behalf, they are required to protect it as permitted by law and contractual obligations.
